During the opening quarter of 2026, Rock Creek Rehabilitation and Healthcare Center was considered the largest nursing home in Rio Grande County, tied with two others—Colorado Veterans Community Living Center at Homelake and River Valley Rehabilitation and Healthcare Center—Centers for Medicare and Medicaid Services (CMS) data shows.

The facility provided 60 beds, which matched the county average in Q1 2026, and cared for an average of 32.8 residents over the period.

Rock Creek Rehabilitation and Healthcare Center received a 2 out of 5 overall rating from the CMS, lower than the statewide average of 3.2 for Colorado nursing homes.

By the end of the first quarter of 2026, Rock Creek Rehabilitation and Healthcare Center had incurred two fines totaling $42,008 and registered two penalties.

There were two for-profit nursing homes operating in Rio Grande County, Colorado.

U.S. Census Bureau estimates show the nation’s older population continues to expand while younger demographics decline. From 2020 to 2024, the group age 65 and older rose by 13%, as the population under 18 decreased by 1.7%. Over the last century, the U.S. older adult population increased significantly—from 4.9 million in 1920 to more than 61 million in 2024.

A study from 2021 reports nearly half of that age bracket, or around 28 million people, may need long-term services and support, and more than a third could eventually enter nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
